General Info

To use the AOC's compensation claim form, please go to the Court's website address: http://www.courtinfo.ca.gov/forms/claimform.htm. From this site you can either fill out the form right there and print it to your printer, or you can download the form to your own computer and use it from your hard drive.

To download the form, right-click on the highlighted claim form link ("in fillable Adobe Acrobat PDF format, (94 KB)") and choose "Save Target As...." You will have the opportunity to choose in which subdirectory of your local computer you want the form saved. Then, whenever you want to use the form, simply click on the name "coaform.pdf" in that subdirectory.

You can also print the instructions for using the claim form from the same webpage.
In order to use the form, you will need to have the Adobe Acrobat Reader on your local computer. If you do not have it, you can follow the instructions on the Court's web page at the following address: http://www.courtinfo.ca.gov/q&a/qa22.htm.
